The right support for complex needs.
CareChoice has been supporting people with complex and high-intensity disability support needs since 2007.

What do we mean by Complex Care?
Complex Care describes disability support that needs more planning, coordination, specialist skills or day-to-day support.
That can look very different from one person to the next.
It might mean several types of support working together. Support Workers with specific training. Clinical or behavioural expertise. A larger care team. Or extra planning around a significant change, such as leaving hospital or moving into a new home.
Complex Care isn’t a separate NDIS funding category. It’s a way of describing the level and combination of support a person may need.
Complex needs don’t make you a complex person.
Your goals, routines, preferences and the life you want should still sit at the centre of how your support works.
Complex care takes a team.
The right support isn’t simply about having more people around you. It’s about having the right people and making sure they work together.
People chosen around you
We match Support Workers to the skills, experience and approach your support requires, with participant-specific training where needed. Our model includes training around individual disability, health needs, emergency plans, manual handling and safe transport where relevant.
A dedicated point of contact
One person who knows what’s going on can make a world of difference when there are a lot of people involved. So for 24/7 complex care programs, CareChoice provides a dedicated Team Leader to oversee the program and act as a point of contact and escalation for the person, family and wider care team.
Clinical and specialist expertise
Support Workers can be backed by Registered Nurses, Behaviour Support Practitioners and other specialist professionals when additional expertise is required.
Support for when things aren’t quite straightforward

There isn’t one diagnosis or checklist that determines whether Complex Care is right for someone.
It may be worth talking to CareChoice if:
- several support requirements need to work together
- Support Workers require specific skills or training
- more intensive day-to-day support is needed
- health, behavioural or clinical needs add complexity
- several professionals are involved in the person’s care
- an existing support arrangement is no longer working.
Not sure whether any of that describes your situation? You don’t need to diagnose your own support needs before you call us.
Tell us what’s happening. We’ll start there.
Different needs call for different kinds of support.
Complex Care can bring several services together. But sometimes one particular kind of support is the best place to start.
