Home Nursing.

Nursing care, without making home feel like a hospital.

Clinical care at home

Depending on your medical needs and approved supports, CareChoice nurses can assist with:

  • medication administration
  • diabetes and insulin management
  • complex wound management
  • complex pain management
  • catheter and stoma care
  • PEG feeding
  • subcutaneous and intramuscular injections
  • oxygen management
  • pressure injury prevention and management
  • monitoring vital signs
  • implementing complex health treatment plans.

All the clinical know-how. None of the hospital corridors.

The right nursing expertise around you.

When someone requires Home Nursing, CareChoice builds a nursing team around their particular medical needs.

We recruit Enrolled Nurses with relevant experience and provide Registered Nurse oversight, while working closely with allied health and other healthcare professionals involved in the person’s care.

Because good nursing isn’t just about knowing the treatment plan. It’s knowing the person it’s for.

Good healthcare is a team sport.

Medication changes. Treatment plans. Allied health recommendations. Pharmacy. Appointments. Quality nursing is complex.

CareChoice nurses can liaise with pharmacies, allied health professionals and the wider healthcare team to help keep clinical care coordinated and everyone working from the same plan.

And if a hospital visit is required, CareChoice’s existing program includes staff accompanying people who need additional reassurance, communication support or advocacy while they’re there.
