Support to get life moving again.
Recovering from a road or workplace injury can change a lot, quickly.

Recovery looks different for everyone. So should the support.
For some people, recovery means short-term help after leaving hospital. For others, it means ongoing attendant care, rehabilitation support or 24-hour assistance.
CareChoice works with you, your family and the professionals involved in your recovery to understand what you need now, where you want to get to and how your support can help you get there.
When things change (as they often do), your support can too.

- neurological conditions
- injuries requiring high-intensity or 24-hour support
- complex needs following hospitalisation or rehabilitation.
Our Support Workers are matched to the individual and can be backed by dedicated Team Leaders, Registered Nurses and specialist expertise where required.
We also work closely with the healthcare and rehabilitation professionals already involved in your recovery, helping everyone stay connected around the same goals.
Ready to leave hospital. Not quite ready to do it alone.
Getting the green light to leave hospital is a big milestone. It can also come with a surprisingly long list of things to work out. What support will you need at home? Is your home ready? Who needs to be involved? And who’s making sure everyone knows what everyone else is doing? That’s where CareChoice can help.

It takes a lot of people to get recovery right.
Occupational Therapists. Physios. Doctors. Speech Pathologists. Case Managers. Rehabilitation teams. Family.
That’s a lot of people who need to be on the same page.
CareChoice works collaboratively with the people already involved in your recovery, keeping communication clear and support coordinated as your needs change.
Experience you can lean on.
Serious injuries can create complex support needs. CareChoice brings together the people, experience and oversight needed to provide safe, consistent support throughout recovery.
