Hospital Discharge Support.

Ready to leave hospital? Let’s get what comes next in place.

Getting the green light to leave hospital is just the beginning.

There can be a lot to work out before someone can actually leave.

Where will they live? What support will they need? Who needs to be involved? And is everything ready for day one?

CareChoice helps bring those pieces together for people who are clinically ready for discharge.

Depending on the person’s circumstances and approved funding, that can include:

  • coordinating with hospital and healthcare teams
  • finding suitable temporary accommodation
  • arranging trained Support Workers
  • establishing 24-hour or complex support where required
  • planning ongoing disability support
  • exploring longer-term housing options.

Less time working out who’s doing what. More focus on moving forward.

A lot of moving parts. One coordinated transition.

Hospital discharge can involve the person, their family, hospital discharge teams, clinicians, allied health professionals, Support Coordinators, funding bodies, housing providers and Support Workers.

That’s quite a group chat!

CareChoice works across the wider team to coordinate the disability support and accommodation needed for the transition, while the treating hospital or clinical team remains responsible for determining when the person is clinically ready for discharge.

Once support is established, it can continue to adapt as the person settles into their new environment and their needs change.

Sometimes the next home isn’t the forever home.

For eligible NDIS participants, CareChoice can help source Medium Term Accommodation (MTA) or other suitable accommodation while longer-term arrangements are being put in place.

If longer-term specialist housing is needed, we can also work with the person and their support team to explore suitable options, including Specialist Disability Accommodation (SDA).

You don’t have to be the person leaving hospital to start the conversation.

We regularly work with:

  • NDIS participants and TAC clients
  • family members and carers
  • Support Coordinators
  • hospital discharge planners
  • healthcare and allied health professionals
  • other people involved in the person’s support.
